What is changing?
Beginning December 1, 2026, PrimeCare of Coral Gables will transition to a membership-based concierge model for patients who wish to continue care with a participating provider. Membership is designed to support a more personal, organized, and relationship-based experience with our practice.
Is the practice moving or closing?
No. PrimeCare will remain in our current Coral Gables location with the same commitment to personal, comprehensive primary care.
ABOUT MEMBERSHIP
What exactly is a membership model or concierge office?
A membership-based, or concierge, medical practice is a primary care model designed around a smaller patient panel and a more personal relationship with the practice. Patients pay a membership fee for non-covered services, amenities, and practice support that help create a more organized, convenient, and relationship-based experience.
What does membership include?
The membership is designed for patients who value continuity, convenience, and a long-term relationship with their provider.
Membership includes non-covered services and amenities designed to support a more personal experience with our practice, such as wellness planning, enhanced communication pathways, administrative support, selected non-covered wellness resources, and practice conveniences.
Medical visits, diagnostic testing, treatment, and other covered services remain separate and are billed to insurance when applicable.

Does membership replace my health insurance?
No. Membership does not replace health insurance. Patients should maintain health insurance for medical visits, labs, imaging, prescriptions, specialist care, hospital care, emergency care, and other covered services.
How do I benefit from joining?
PrimeCare membership is designed for patients who want a more personal and organized relationship with their practice. Members receive a more personalized experience, including more time with the care team, same day or next day appointments when available, 24/7 communication (ie. text, email, calls), after-hours guidance for urgent concerns and more.

How can I contact the care team as a member?
Members will receive clear instructions for contacting the practice through approved communication channels, which may include phone, secure messaging, text, or email. Non-urgent messages should be sent during regular business hours, while urgent after-hours concerns may be directed through the appropriate membership communication pathway. Emergencies should always be directed to 911 or the nearest emergency department.

How does the membership fee work?
Membership may be paid annually or monthly. The membership program begins December 1, 2026. More information about payment timing, cancellation terms, and membership details are described in the Membership Agreement. You can setup a time to learn more about the membership program by clicking here.
Can I use HSA/FSA/HRA funds?
Some patients may choose to use HSA, FSA, or HRA funds, but eligibility depends on individual plan rules and tax guidance. PrimeCare cannot guarantee that membership fees will qualify for reimbursement. Patients should confirm with their tax advisor or plan administrator.
What if I decide I want to cancel my membership?
We understand that membership may not be the right fit for everyone. Cancellation terms are described in the Membership Agreement. Our team can review those terms with you before enrollment. We will provide a pro-rated refund although limitations apply and we recommend reviewing your Membership Agreement for details.
What if I do not want to join?
We understand and respect that membership may not be the right choice for every patient. Beginning December 1, 2026, ongoing care with a participating PrimeCare provider will be available only to enrolled members.
For patients who do not enroll, our team will help support a smooth transition to another primary care provider, including assisting with medical record transfers upon request.
